How Surgicure is rethinking airway management

In modern medicine, some of the most consequential failures stem not from a lack of innovation, but from the persistence of outdated standards. Airway securement—the act of keeping life-sustaining breathing tubes in place in critically ill patients—is one such case.

Each year, tens of thousands of patients experience life-threatening complications when breathing tubes shift or are unintentionally removed—events associated with mortality rates approaching 30%. At the same time, legacy securement methods—primarily adhesive tape and facial-based devices—can increase the risk of device-related pressure injuries by as much as threefold, with pressure injuries themselves linked to up to a fivefold increase in mortality. These complications are not only clinically devastating, but financially punitive: many are classified as “Never Events,” leaving hospitals to absorb the cost and contributing to more than $8 billion annually in avoidable U.S. healthcare spend.

Critically, these same devices restrict access to the oral cavity, limiting compliance with infection-prevention guidelines from ICHE and NPIAP, where oral care is central to reducing ventilator-associated pneumonia (VAP). In military medicine—where airway complications account for more than half of combat casualty care issues—this remains a top operational priority. Yet despite clear clinical and economic incentives, the standard of care has remained largely unchanged.

Surgicure Technologies was founded to close this gap. Its flagship product, the HORSESHOE™ airway securement platform, originated from U.S. Army respiratory therapists at Brooke Army Medical Center, who repeatedly saw tape-based securement fail in combat and austere environments. Rather than iterating on adhesive-based solutions, the device introduces a fundamentally different approach—anchoring the airway intraorally using natural jaw biomechanics.

By securing to the molars—the most stable structure in the jaw—the HORSESHOE™ eliminates reliance on adhesives and addresses the root cause of device-related pressure injuries, while maintaining consistent tube position under motion, as demonstrated in frontline deployment. Because it does not depend on facial fixation, it preserves full access to the oral cavity—supporting oral care protocols tied to infection prevention and enabling surgical, dental, and imaging workflows that traditional methods obstruct. The result is a system that improves clinical outcomes while reducing workflow inefficiencies across care teams.

Over the past year, Surgicure has rapidly transitioned from validation to early commercialization. The device is a Class I, 510(k)-exempt product manufactured under an ISO 13485–certified quality system, with completed mechanical, biocompatibility, human factors, and cadaveric validation. Early clinical use at Massachusetts General Hospital has demonstrated strong safety and usability outcomes, while deployment in frontline settings has validated performance under real-world conditions. This clinical momentum is translating into commercial traction. Exclusive partnerships with MetroMed and MED Alliance now extend distribution coverage to approximately 60% of the U.S. market, with additional strategic partnerships under discussion.
